Ammalato functions as both noun and adjective. As a noun, l'ammalato / gli ammalati refers to a sick person or patient; as an adjective, essere ammalato means to be ill. The feminine is ammalata. Note that in Italian you 'are sick' (sono ammalato) rather than 'have an illness' as the default construction — unlike the verb-based avere la febbre (to have a fever).
