Midollo has two main uses: midollo osseo (bone marrow) in medicine, and midollo spinale (spinal cord). Its plural is irregular and gender-switching: le midolla (poetic/archaic) or more commonly i midolli. Do not confuse with zucchino or zucca — the vegetable 'marrow' is zucchina/zucchino in Italian.
